Host a Vision Clinic
Our goal is to make hosting a vision clinic at your school as easy as possible. At this point, you have also taken the initiative to get your students much needed vision care, so let’s talk about what that means.
As the host school, you will:
Provide space in your building for a volunteer Doctor(s) of Optometry and their staff to provide comprehensive eye exams.
Identify and ensure eligibility of students participating in the clinic.
Take care of scheduling the students for exams, and manage student movement to and from the exam location.
We are working on implementing an electronic system to transmit forms, but for now, we need you to make sure all forms are collected from families, including an opt-in medical consent form.
